HIPAA Privacy Notice

This notice describes how medical information about you may be used and disclosed, how you can access this information, and the privacy responsibilities of L&P Aesthetics Medical, Inc. Please review it carefully.

This HIPAA Privacy Notice applies to protected health information handled in connection with clinical care. Personal information collected through the L&P Skin Shop ecommerce store is addressed separately in the L&P Skin Shop Privacy Policy.

Your rights

You have the right to:

  • inspect or obtain a paper or electronic copy of your medical record, subject to legal limitations;
  • ask us to correct health information you believe is incorrect or incomplete;
  • request confidential communications or ask us to contact you in a specific way or at a specific place;
  • ask us to limit certain uses or disclosures, although we are not always required to agree;
  • receive an accounting of certain disclosures of your health information;
  • obtain a paper copy of this notice at any time;
  • choose a personal representative to act for you when legally authorized; and
  • file a complaint without retaliation if you believe your privacy rights were violated.

Your choices

For certain health information, you may tell us your preferences about what we share. These choices may include whether we:

  • share information with family, friends, or others involved in your care;
  • share information in a disaster-relief situation;
  • contact you about fundraising; or
  • use information for marketing or sell protected health information.

If you cannot tell us your preference, we may share information when we determine it is in your best interest and permitted by law, such as during an emergency. We will obtain written authorization when HIPAA requires it, including for most uses of psychotherapy notes, most marketing uses, and sales of protected health information. You may revoke an authorization in writing, except to the extent we already relied on it.

How we may use and disclose health information

HIPAA permits us to use or disclose protected health information without written authorization in certain circumstances, including to:

  • treat you and coordinate your care;
  • bill for services and obtain payment;
  • operate our organization, improve care, train staff, and manage quality and compliance;
  • help with public-health and safety activities;
  • conduct research when legal requirements are satisfied;
  • comply with law and health-oversight activities;
  • respond to organ and tissue donation requests;
  • work with coroners, medical examiners, and funeral directors;
  • address workers’ compensation, law-enforcement, and other government requests; and
  • respond to lawsuits, court orders, and legal proceedings as permitted by law.

Other uses and disclosures not described in this notice will be made only with your written authorization unless law permits or requires otherwise.

Our responsibilities

  • We are required by law to maintain the privacy and security of protected health information.
  • We will notify affected individuals as required if a breach may have compromised the privacy or security of their information.
  • We must follow the duties and privacy practices described in the notice currently in effect.
  • We will not use or share information other than as described here unless you authorize us in writing or law permits or requires it.
  • We will accommodate reasonable requests and restrictions when required by law.

Changes to this notice

We may change the terms of this notice and make the revised notice effective for health information we already maintain and information we receive in the future. The current notice will be available on our website and upon request.
